Parental Rights; Requires parental consent for a minor's treatment for certain diseases; revises exceptions for certain parental rights; creates parental right to review & consent to survey or questionnaire provided to his or her minor child; creates parental right to grant permission for results or responses of such survey or questionnaire to be shared or distributed; revises exceptions for specified requirements of parental consent; prohibits use of biofeedback device on minor child without written permission from minor child's parent or guardian; defines "biofeedback device"; requires results of use of such device be provided to parent or guardian; requires that such results be held as confidential medical record.
